The CCDC 9B – 2018 Statutory Declaration of Progress Payment Distribution by Subcontractor is a formal, legally binding document used in the Canadian construction industry. It's a sworn statement made by a subcontractor as a condition for receiving either:

The Ontario Student Assistance Program (OSAP) may require you to provide information about your marital status as part of the application process. This information is generally used to assess your eligibility for financial assistance. In general, when marital status information is required for OSAP or any other financial aid application, you may be asked to provide supporting documents or complete an affidavit confirming your marital status. You will be asked to affirm that the information you are providing is true. The affidavit must be signed under oath before a notary public or commissioner of oaths.
